The presence of cyanosis indicates either an associated cardiac anomaly, such as tetralogy of Fallot, or a marked increase in pulmonary vascular resistance, either from progressive sclerosis of the pulmonary vascular resistance, either from progressive sclerosis of the pulmonary arteriolar bed or from congestive heart failure.
